At one point, when training Lessa to go _between_, F'lar warns her
that:
'"once we came across a Dragon and a Rider entombed together in solid
rock. They...were...very young." His eyes was bleak.'
The interesting thing about that is how odd it is. If F'lar meant
that the dead pair had materialized inside a solid object, then that
should either have set of a major explosion of some sort, or else when
a Dragon arrives, whatever matter is in its arrival point vanishes
_between_.
This last is almost certainly true, otherwise every time a Dragon
materialized, even in the air, it and its Rider would likely die of
air bubbles forming within their bodies.
But if that's the case, then the Dragon and its Rider would have found
themselves entombed on all sides by rock, perfectly fitting their
form. If they reacted fast enough, they could teleport out again.
Granted they may have panicked, but still...
Also, the Dragon should have been able to live for the few seconds
necessary to communicate its distress to the other Dragons. They can
communicate all over the planet, after all. So even if no help could
come, the Riders should still have known roughly what happened.
Of course, they may _have_, and the records were lost by F'lar's time,
too. Certainly his words to Lessa implied that they had founded the
entombed pair as a surprise.
