They are written in different scripts.
The grammar is exactly the same. There is no difference.
There is a set of common words in both Hindi and Urdu. There are other
words that are not common to them, but it depends on the speaker's
background. One speaker may use more Persian and Arabic words while
speaking Hindi than another. Hindi speakers may even refer to these words
as "Urdu words".
Think of American English and British English, written in two different
scripts, with the users not mixing with each other over a long period of
time. Same grammar, slightly different vocabulary.

"peace" == शान्ति shaanti in Sanskrit
अमन aman in Urdu. But then, also in in Hindi.
