I've searched for a common name for this Czech model for a long time.
She seemed to have different names on each website and each magazine
she appeared in. But finally I had luck to stumble upon this site:

which reveals her name (or at least model name) was Alena W.
She seemed to have been active 1994 - 2000 so pictures of her are a
