The 24-hour radioactive iodine uptake remains a very useful clinical tool f
or evaluating the thyroid gland; however, the normal values have changed ov
er the years. The traditional values of 15% to 45% are clearly not applicab
le today and the revised values of 9% to 32% are suspect in some areas wher
e iodine content is rapidly changing in the general diet. The values in a r
elatively stable population in Ft. Smith, Arkansas, without significant acc
ess to a continuous-batch bread with a high iodine content do confirm the n
ational trend of the early 1970's suggesting an overall normal range from 1
1% to 30%. Values in men were found to be lower with a range from 11% to 23
%, whereas in women the values ranged from 13% to 32%. Each nuclear medicin
e department should be responsible for establishing its own normal range an
d reevaluating it on a fairly routine basis-certainly whenever known dietar

The 24-hour radioactive iodine uptake remains a very useful clinical tool f
or evaluating the thyroid gland; however, the normal values have changed ov
er the years. The traditional values of 15% to 45% are clearly not applicab
le today and the revised values of 9% to 32% are suspect in some areas wher
e iodine content is rapidly changing in the general diet. The values in a r
elatively stable population in Ft. Smith, Arkansas, without significant acc
ess to a continuous-batch bread with a high iodine content do confirm the n
ational trend of the early 1970's suggesting an overall normal range from 1
1% to 30%. Values in men were found to be lower with a range from 11% to 23
%, whereas in women the values ranged from 13% to 32%. Each nuclear medicin
e department should be responsible for establishing its own normal range an
d reevaluating it on a fairly routine basis-certainly whenever known dietar
y iodine factors change.

intended for the detection and diagnosis of Conversion hysteria. Hathaway
and McKinley used 50 patients with a clinical diagnosis of psychoneurosis,
most of whom were believed to have a conversion disorder.2 Butcher and Will
iams describe patients with a conversion disorder as exhibiting personaliti
es characterized by denial and flamboyant social interactions. These indivi
duals may react to stress with the development of a physical complaint.3 Th
e original understanding of a conversion reaction was believed to be relate
d to a metaphorical resolution of an intrapsychic conflict (primary gain) t
hat may be reinforced by attention or the removal of undesired responsibili
ties because of the physical problem (secondary gain). Although individuals
with an elevated Scale 3 tend to react to stress with physical symptoms, t
hey often have limited insight into their feelings and motivations.2,3 Furt
her, these individuals may frequently need approval and attention from othe
rs.2 Elevations on this scale may reflect a difficulty acknowledging or acc
epting aggressive or hostile feelings. Therefore, hostility may be expresse
d through indirect or passive means. Although these individuals may demonst
rate good interpersonal skills, their ability to form rewarding, reciprocal
, and mature relationships is likely to be impaired.3

A minority of patients with chronic pain do fulfil the criteria for the dia
gnosis of a somatoform disorder. In such conditions there is continued pres
entation of physical symptoms together with persistent requests for medical
investigations despite negative findings of organic illness and reassuranc
e by doctors that the symptoms have no physical basis. The diagnosis par ex
cellence of a somatoform disorder is somatisation disorder, where pain is j
ust one of many symptoms exhibited by the (usually) female patient. This di
agnosis is not common, ranging from 0.2% of patients referred to a liaison
psychiatry service (Smith et al, 2000) to 5% of medical patients (Fink et a
l, 2004). This figure is higher than the previous figure of Smith et al (20
00) because Fink et al (2004) used ICD-10 criteria. The ICD-10 diagnosis of
persistent somatoform pain disorder was 1.5% in this same population (Fink
et al, 2004). This low figure is not too surprising, as the latter diagnos
is can be made only if the pain described by the patient ' occurs in assoc
iation with emotional conflict or psychosocial problems that are sufficient
to allow the conclusion that they are the main causative influences' (Worl
d Health Organization, 1992: p. 168). Contrast this with the diagnosis of p
ain disorder listed in the somatoform disorders section in DSM-IV (American
Psychiatric Association, 1994). For this diagnosis to be made, 'psychologi
cal factors are judged to have an important role in the onset, severity, ex
acerbation, or maintenance of the pain' (p. 461). Although cases largely du
e to physical illness are excluded, as also are cases where the pain is 'be
tter accounted for by a mood, anxiety or psychotic disorder', more cases wi
th pain and emotional sequelae achieve this level of diagnosis on the DSM-I
V schedule than on ICD-10.
The differentiation of the somatoform disorders has been questioned (Wessel
y et al, 1999). The problem with the diagnosis of somatoform disorders in g
eneral and of somatoform pain disorders in particular is the judgement requ
ired that the symptoms manifest are due to psychological factors. It is not
easy to determine this objectively and most psychiatrists working in the a
rea are aware that physical and psychological factors both contribute signi
ficantly to the presentation (Mayou, 1991; Merskey, 2000). The evidence of
psychological causation cannot be assumed from a history of previous risk f
actors. The reporting of unexplained pain symptoms as due to previously exp
erienced psychological trauma has been found to be an artefact of retrospec
tive self-report rather than a consequence of actual events (Raphael et al,
2001). This being said, there is evidence that some non-organic pains aris
ing in adolescence have a psychogenic basis (Hotopf et al, 1999).
The value of the present classifications of these syndromes has been brough
t into question because of the imprecise categorisation of such disorders a
nd the fact that many patients fall into the category of undifferentiated s
omatoform disorder, a watered-down version of somatisation disorder (Bass e
t al, 2001; Sharpe & Mayou, 2004). Dimensional assessment of pain on the ax
es of nociception, evaluation of pain, mood consequences of the pain and pa
in behaviour (Karoly & Jensen, 1987; James, 1992) may be of greater clinica
l relevance.
