It's happened on at least one DC book I'm aware of - Justice League of
America (1974) 111, where the digital version included both parts of
the Seven Soldiers of Victory story (the original paper version spilt
the story between 111 and 112).
DC is especially bad at releasing incomplete/botched digital editions
(most of their Golden Age releases eliminate most of the stories
besides the cover feature); Marvel's seem to be fairly decent/complete
for the most part, even including the letters pages in some cases. (I'd
prefer they were there in all cases, but I accept I'm a freak in this
respect - possibly others as well, but I digress.)
